MMS Community Care Foundation was created to help bridge the gap between healthcare, community resources, and real-life family support.

Our foundation exists to support seniors, caregivers, disabled adults, and vulnerable families through education, outreach, advocacy, wellness programs, and compassionate community care initiatives throughout Eastern North Carolina.

We believe no family should have to navigate aging, caregiving, or community support alone.

What We Do

Caregiver Support Services

Caregiving can be overwhelming emotionally, financially, and physically. We provide programs designed to support and encourage caregivers through education, resources, and community connection.

Senior Wellness & Check-In Programs
